There are no other appeal forms available and no other option listed to ask for re-enabling the monetization.

It doesn’t :( there are 2 appeal forms on that help page. I’ve submitted both. The response I got did not clarify why it was disabled nor enabled it. The appeal forms are for violating community guidelines & invalid clicks. I’ve had neither.pic.twitter.com/3741ZVNID1
