It's just how much physical space the card itself takes up. You won't be using the next two slots under your 16x slot, but if you have a board with 16x 1x 1x 16x, you keep half a slot of space between top card and bottom card in multi gpu configs.

The cooler looks the same width as a standard 2 slot, just with some additional clearance. I am having trouble seeing the point. Maybe just the pictures are at a bad angle.
