1/? I’ve been getting questions/ concerns lately, including yesterday at @ClevelandClinic, about the @CDCgov pain guidelines, and unintended impact on patients they were never intended for. Providers and hospitals, PLEASE read this article!https://www.nejm.org/doi/full/10.1056/NEJMp1904190 …

2/? “Unfortunately, some policies and practices purportedly derived from the guideline have in fact been inconsistent with, and often go beyond, its recommendations.”

3/? A consensus panel highlighted these inconsistencies including inflexible application of recommended dosage/duration thresholds & policies encouraging hard limits & abrupt tapering of drug dosages, causing sudden opioid discontinuation or dismissal of patients from a practice.

3/? The panel also noted the potential for misapplication of the recommendations to populations outside the scope of the guideline, eg sickle cell crises, advanced cancer, & perioperative setting.

4/? Opioids are STILL being overprescribed and inappropriately prescribed to too many patients, and the @CDCgov guidelines are an important way to address this. BUT... we MUST NOT hurt some patients in the process by inappropriately applying the guidelines.

5/ Again, providers/ prescribers/ policy makers, PLEASE read the above article. We must ⬇️ inappropriate prescribing, but we CAN do this without hurting patients the guidelines were NEVER meant to apply to! 🙏🏽pic.twitter.com/4MjJfsXiQp

The @DEAHQ and Attorney State Generals @NatlAssnAttysGn are still going after drs for prescribing higher than 90mme what is going to be done about that @Surgeon_General ???

Why won’t the DEA publish, in front of everyone, what they are targeting doctors for specifically?

Hospitals have been put into impossible positions. Remember, JC made “PAIN” the 5th vital sign, PG surveys expected “No Pain”, & @CMSGov waded in?

That’s what helped fuel today’s crisis. Patients were literally told they should expect to be pain free. On D/C it was like a goodie bag of pills.

Thank you for speaking up. Sadly, it's too late. So many have died. Many more suffering horribly. Must repeal guidelines and stop DEA. PLEASE stop the insanity. At a minimum, hold a press conference and meet with DEA/Medical Boards/Law makers. No more suffering...PLEASE.

Why was I tapered from 6mme to 3mme and why did they remove my effective benzo, of which I only took 0-3/day and replace it with "pez"? I never should have been touched. I am not alone. Please, if this is real, continue to help us and stop the DEA.

I have rare diseases that cause pain AND muscle cramps, vibrations, tetany ... I can hardly sleep now that I can't have benzos while having lowered my opiates by over 90% & going bankrupt buying MMJ. Lowering my life-sustaining supplements until my heart stops. Are ya happy now?

Two points: #4 is simply not true; the number of prescriptions for opioids has been on the decline for years & yet opioid related overdoses have been skyrocketing. People are being drastically tapered & cut off of their opioid pain Rx. The problem is heroin & illicit fentanyl.

Are you also making certain to address this with prescribers? Cardiac arrests due to untreated pain are a known potential deaths, as is attempted suicide from unbearable pain and no quality of life.
