James, respectfully, that is categorically untrue. 1) Unprecedented situation 2) Public health experts know case tracking/ confirmation takes time- esp in rural areas. 3) CDC research shows many/ most spread actually/ already occurred before initial case diagnosed.
It’s important that people understand HIV diagnosis and treatment in rural areas very different than in urban. Stigma and other factors play a big role. I am fighting to ensure rural (as well as urban) America is represented in our current policies and actions.
But I 
how so many people who have never been to Scott County or tried to implement public health interventions in rural communities- much less ones involving stigmatized diseases, unheard of outbreaks, & controversial interventions, weigh in so definitively on the response.
As a biostatistician from rural Kentucky, with family just a county over from the outbreak, that has helped with on-the-ground HIV prevention efforts back home, I feel uniquely qualified in telling you, the surgeon general, to delete this awful condescending tweet.
