If I kill 100 people in the name of christ and believe that's why I'm doing it, and use a justification rooted in the bible, then "not really a christian" is a difference without distinction.

"thou shalt not kill", "judgment is mine, sayeth the Lord" etc. etc. - There is no justification in the Bible for killing a 100 people. People make shit up all the time, doesn't mean they are right.
