In short, he had a column years ago where he talked about how awful it was that insurance companies used the term "medical costs". Except: a) There's nothing wrong with calling costs that are medical 'medical costs', and; b) Insurance companies don't use that term.

In insurance accounting, 'costs' generally applies to administrative stuff, payouts are called 'losses'.
