the only cue we have for what he looks like is also a statue so there's a possibility the Primus doesn't look EXACTLY like that, or that the nature of the Maw can twist the appearances of the Eternal One
Both of the imageries of the Primus doesn't look entirely accurate to each other. And one has to consider, "Hey in Maldraxxus your body changes quite a bit." So I would wager the Primus being as ancient as he is probably has gone through different body parts a lot.

that too, I feel like minor differences in the Runecarver's appearance in comparison to the statue of the Primus aren't enough to sink the theory. Everything just adds up too perfectly for him not to be.
