#2 would mean it literally is just High King 2. We go from blizzard trying to make a Blue Warchief in MoP to a Red High King.
Certainly seems that could be the way they're going. Also could mean we could see Thrall vs Turalyon at some point then. Which in my eyes is like Lothar and Orgrim's successors duking it out.

Well, Orgrim was Thrall's mentor. Turalyon hasn't mentored anyone. So for Thrall, it would be a pretty personal and interesting fight for him to face off against the one who defeated his mentor Orgrim.
