I think people confuse Cairne and Garrosh's Mak'gora as the default version of the Mak'gora when you don't set rules. When that's not the case at all. I personally think Thrall and Garrosh just have their own ones set since Wrath or they simply don't have rules established.

AFAIK the major change to happen to the mak'gora custom is that, under Thrall, it was no longer a duel to the death. This is also what Garrosh meant, because he was unaware of the other accompanying customs (having witnesses/blessings etc).
