There isn't a default setting to Mak'goras. Thrall and Garrosh have fought multiple times now with him using the elements and Garrosh still held his own. In Orgrimmar (TWICE) Garrosh and Thrall fought with elements still involved and in one of those was a Mak'gora.

In Thrall's case against Garrosh's two weapons, he wielded Doomhammer and Elements in the first fight, that's equalizing the count as an Enhancement Shaman. But in the final fight, he started only with Doomhammer against Garrosh's Gorehowl and never used Elements until the end.
I don't think the two have to match-up with what they're fighting with. Multiple times Thrall and Garrosh have fought each other inside and outside of a Mak'gora with the elements still playing a part. And in those matches, Garrosh still came close to winning. (1/2)

Garrosh fully well knew that Thrall was going to wield the elements again in their final battle in AU Nagrand and probably thought once again he'd win and this time end Thrall. If he truly didn't want Thrall to wield the elements we both know Thrall would abide by that. (2/2)
