He is indeed all 3, but the Orcish loyality he has is always overlooked. Garrosh cares absolutely nothing about himself (unlike Sylvanas). For Garrosh the Orcish people is everything for him. As a relentless tyrant, warlord and Warchief.. he stops at nothing for his people.
I see her as having something akin to PTSD; becoming unhinged by what happened at Arthas' hands. Asking her to live by our standards would be like telling a schizophrenic to 'get over it'
The issues with Garrosh is he began to ignore his own people on what they think and began to basically put his thoughts as their's. Its why we had some orcs agree with Garrosh and others forced to fight/rebel against him.
