Deathwing had more of a connection to Azeroth and its Elementals than Sargeras ever did. While Sargeras' sword has put a lot of damage on Azeroth and her heart it didn't dig deep enough to cause a Cataclysm. However if her wounds remain as they are the planet will shatter.

Well the Maelstrom is already a wound so probably. But Silithus was her heart.
