Dreadlords held immense control over the Scourge able to almost overpower the Lich King's grip. We know that Mal'ganis is still out there (from the Broken Shore) one of his lines directed to Zul'jin in HotS: Mal'Ganis: What makes you think we weren't your "loa" to begin with?
Potentially the Legion (aka) the Dreadlords or something entirely new probably did it. As for who resurrected him my bet is on Rezan just before he died.
