Small chucks of ceramic can completely shatter tempered glass, called ninja rocks, and can be obtained from spark plugs, and some dishware.... that might have done it. Or tempered glass can literally just spontaneously explode, from imperfections in the glass. glad your alright

Yeah, I think heat resistant glass has been commonly made out of cheaper, less heat resistant glass for decades, leading to this issue with pyrex dishes and the like.
