Choanal atresia is when soft tissue (or bone) blocks the nasal passage. It happens in about 1 in 7000 live births. Clinicians make the diagnosis when a neonate has an increased work of breathing and/or the inability to feed + difficulty/inability passing a nasal catheter.

Although less common, Pyriform Aperture Stenosis belongs on the differential. In babies with mild PAS, passing a catheter may be possible & despite increased WOB, they tolerate feeding in brief intervals. They often present to pediatrician with poor feeding failure to thrive.
