Competitive pressures do not work to lower pharma pricing. Healthcare is subject to inelastic demand - if I need X to live, I will find a way to pay whatever X costs so I can stay alive.

Yeah, but also find a way to lower malpractice insurance. I think insurance is a great idea... but unreasonable lawsuits or unwilling to pay far settlements have turned into cash cows for the people with the most expensive lawyers.
