once at my last job, I snuck a small change in, trying to fix a bug, without doing full regression tests, and when it went live it broke the system for a major customer, and I got paged at like 3 AM to log on remotely and fix it.
that was one terrible mistake that shouldn't have cost me my job, and didn't, because I took responsibility and made sure I fixed it carefully and correctly, and from then on made extremely sure not to skip steps or avoid process, because the process is there for a reason.
