you have to be a legal adult in order to sign a contract for cellphone service, don't you? (bear in mind I'm old, cellphones weren't ubiquitous before I could buy whiskey and smartphones didn't even exist until I was nearly thirty.)

which is to say, it seems to me that if they weren't cowards and/or villains, app store/platform owners like apple and google would simply require the primary account holder to confirm on each device whether that device is used by an adult or not, and age-gate content accordingly

would horny and/or curious teens find ways around this? of course they would, as they always have with every kind of age-gating approach throughout history. basically all of those teens turn out fine.

the idea that in order to raise healthy children we must sand all culture, targeted to adults or not, down to a level appropriate and "safe" for kids is extremely harmful.

let adults make art that isn't for children. it's fine. it's good. even if some teens end up seeing it, they'll be fine. make sure they have adults in their lives they can trust and safely talk to about it.
