There's something deeply unexplained about the U.S. coronavirus death rates compared to other countries. It can't be explained by hospital payment incentives or even leadership mistakes. For some reason, we are all blind to the biggest variable, whatever it is.

Which death rates do you refer to? If it's the deaths per capita, then the main reason are the many infections per capita in the US. If it's the deaths per infected in a specific age group, then the differences to other Western countries aren't big.

I realize infections cause deaths, but why so many of both categories per capita here? Unexplained.

IFR (infection fatality rate) in the US is not materially different from other developed countries with similar age structure. IFR in US is estimated at 0.6%-0.8%, in Europe 0.3-0.8%. High number of obese in US may explain this.

Are you sure there's a mystery? I think it's pretty obvious that the mitigation of the pandemic has failed across the US for a variety of reasons. If I were to come up with a timeline, it might look like:

          1. Initial hit was bad due to lack of tests, so a long unmonitored initial spread. 2. Tests still lacking, lockdowns couldn't be timed and targeted where necessary. States had to reopen before capacities and strategies for reopening were in place or respected. Renewed spread.

Perhaps because Americans have politicised and ignored preventative measures (eg masks and lockdowns) that are well accepted elsewhere, higher community transmission rates have given more people high viral loads and, therefore, more severe disease?https://www.medpagetoday.com/infectiousdisease/covid19/88692 …

But have we compared to other countries doing much better? I doubt it.

The society with such a focus on liberty and freedom. It is a great thing, but when a pandemic happens, there isn't much control over the populace to enforce lockdowns, mask orders, etc. and it spreads.

We are one country that is only slightly smaller than continental Europe. All of the implications of that explain much of this. We need to keep that perspective in mind.

Exactly! But it's almost like we are 50 different countries as well...
