Does anyone have a source for how German doctors are prescribing HCQ, if they are? Google is suspiciously silent.

Confirmed cases in Germany are 0.27% of the total population. So the chances that my doctor has seen a COVID-19 case are actually quite low. But I can try.

This makes me think about that statistic for other countries. In the USA, nationwide confirmed cases are only 1.7% (skewed by hotspots), so is it true that most doctors in the US also probably have not seen a COVID-19 case? (although higher likelihood than Germany)

The suppliers would have a general idea of how much is prescribed based on sales. They have their historical numbers and know how much of an increase, if any, there has been since Wuhan virus spread.
