Aside from the age issue in Italy, they do have a different medical system than we have. Not all areas even have hospitals

Italy had a very old population. 3 generations often live in the same home. The young get the disease (minor inconvenience for them) and spread it to the elderly grandparents they live with. Plus, their healthcare system is simply not as good as ours. Iran is a mess in many ways
