An isolated COVID19 infected patient becomes more dangerous to caretakers as a function of disease severity (I assume because they cough and breathe out more viruses). A non symptomatic infectee is less dangerous. A loose coughing infectee is deadly. This is why masks help.

A virus with a reproduction number of more than 1 will spread, but less than 1 will stop. So small differences to reduce spread can be huge. The R0 for something like flu is often something like 1.3. For 1.3, once 25% are vaccinated/immune, it should stop on its own.
