We are not however in complete ignorance regarding the importance of keeping this particular prisoner alive. How hard can that be? No cameras? How can that be? Less than 3 weeks after he either attempted suicide or was attacked? How can that be?

That's what you get wrong. They were reportedly understaffed so they had hundreds of jobs, not one. Someone was going to be unwatched. Probably lots of prisoners were.
