As long as the premium option is guaranteed to exist I could see it working... But you'd have to deal with poor people in worse health making up the brunt of the free healthcare. Not sure that actuarially you can make it work. There would have to be massive controls in place.

So when the wealthy are buying premiums that cover their hip replacement at 80yrs of age, while poor and middle class are rationed out of that option, will a dual system be considered "fair"?

The dual system doesn't solve the problem of govt efficiency. Govt run healthcare is guaranteed to "generate revenue" because tax dollars would be block allocated to fund it. The govt still doesn't need to compete, so there's no incentive to do it at the lowest possible cost.
