Depending on what laws govern in particular state or DC, anyone or any relative or close associate can go to court to get an involuntary evaluation. Doctors and some other medical personnel can generally order one as well. With POTUS, cabinet also has this power (25th amendment)

Would be in country's best interest for someone less politically connected than Pelosi to do this. If Trump's family cared about him, would be done by them. If GOP cared about surviving Trump, would have done it (they don't yet realized the damage they've done with con voters)
