If you were designing ASCII today, why wouldn’t you put ‘a’ at 0?

I think the entire store allocated for the string should be zero. If the string fills the capacity allocated, it won't be zero terminated but you have a struct that stores the used length, length capacity (how much was allocated) and the pointer to the memory. And copy on write.
