I understand and agree, I'm just wary of how we so easily fall into the narrative structure of "we found a bad guy", *fired*, "fixed!"

I don't want to see the public firing of one person and chastising of some direct enablers, I want to see a policy and goals written down...

Agree completely. Policy also needs to be written down to mitigate company's fear of wrongful termination lawsuits.
