This is the question that I have. Has the window of appropriateness for gender specific titles/honorifics passed by? When initiating communication outside your immediate professional circle? Even if you are fairly confident you know the party’s asserted gender identity?

Depends on the recipient, but I think there's a much higher risk of legitimately* offending someone by using them than by omitting them.

By "legitimately" I mean things like misgendering, perceiving ma'am as condescending, etc. as opposed to "zomg pc run amok!!11" offense.
