1) Dial 1-800-240-8100 2) Press 1 for English or 2 for Español 3) Press 2 for lost or stolen card 4) Press 1 for I received a card 5) Enter last 6 digits of SS # 6) Enter zip code 7) Listen to recorded message 8) Press 1 to speak with a customer service rep
