Cost limits access for some patients. And even when patients pay nothing for PrEP, someone is paying for it, including other patients in the form of higher premiums. Cost is relevant from a public health perspective and I think should be considered in prescribing decisions.

Agree and the cost in the US is outrageous and a major barrier to wide scale uptake

Once TDF is generic, there is really no reason to use TAF except in special circumstances (pre-existing kidney disease etc). Don't think anyone has done it but I expect the trade-off with cost and effectiveness for TDF vs. TAF won't show TAF in a flattering light.
