there are times when i want what i say to be taken very literally and there are times when i want what i say to be taken very metaphorically / poetically / symbolically and i haven't figured out how to communicate accurately which times are which

we used to have this rule that poems are supposed to rhyme & it worked great

I think you should have a sign with you at all times. One side has a picture of a medical textbook, the other is a unicorn. Then you just hold it up after you are done speaking with the right side facing your audience.
