I HAVE THIS EXACT problem and I have the internal Elgato card in my computer. I think the way people got around it was by capturing the software itself with its live-playback vs. actually emebedding it into the OBS software, I think when you do that it just desyncs no matter what
I've done the same. Instead of making the Elgato a video capture source in OBS, I screen capture the Elgato software.
This looks like a common solution for a lot of people. Is there any loss of quality when you do this, or is it not noticeable?

I haven't done it myself yet, been just disabling / re-enabling audio to resync every 30-40 min - but will likely make the switch over to this workaround because when I play with the card w/out OBS it never desyncs
I’ve been struggling to get obs to even recognize my elgato as a capture device, I’m going to have to try this tho
