it’s rather mysterious to me, though. As far as I can tell a large majority of people are barely in contact in with anyone outside of their households. If it’s only “essential” workers who are generating new cases, why isn’t that being reported?

The "large minority" is not enough. 6ft is not enough. Not testing supply workers is not enough. Sending infected people back home into their apartment building is not enough. Hotspots need to be completely quarantined.

The ship has sailed on strict lockdowns in the US. I also doubt they would be possible to re-implement in Italy or Spain.

This will mean that unless there is an effective vaccine, people that can afford it will move to regions that implement effective lockdowns, leading to an incredible economic and health divide.

Most US airlines still don't require mandatory face masks for their passengers. A lot of those passengers are health care workers that likely have not been tested. It is no surprise that the virus is spreading all over the country.
