For most adults, a good case can be made to remove all restrictions on buying prescription medicines. The rules are designed to protect a minority of irresponsible people. What would a system look like that grants freedoms and choices based on ability to take responsibility?

I don't know. You could in principle use tests, similar to a drivers license, and a process by which the license can be revoked if you turn out to be irresponsible or become senile?

That is not quite the criterion. In the same way as all adults are allowed to buy chainsaws but leave the more complicated chainsawing decisions to experienced professionals, we can expect that responsible adults will consult an expert before using dangerous medications.
