Another problem is that these internal states are inaccessible to others and opaque. A well-trained, intelligent doctor may be clinging to dualism not for lack of understanding, but out of fear of the alternative. Where is the responsibility then?

It's not usually Fear that motivates dualism -just as in scientists it's not just fear that motivates strict reductionism or inferential reasoning.

Well I definitely agree with all that. I think what we’re talking about is the validity of the idea of responsibility in a situation of very low to zero “free will”. It’s a bit of a paradox, which is why I agree+disagree with you

Taken to its extreme, you don’t have the agency to convince me (if I won’t be convinced) and I don’t have the agency to change my mind; further: there’s no point to conversation. Clearly we don’t believe that. The middle ground is unclear, however.
