It's dark souls 1 with the DLC. And as @ObeyAtaraxia said get DSFix, helps a lot since it's a bad pc port
Should I play it with a ps4 controller, xbox controller or mouse and keyboard?

if you like to torture yourself go ahead with mouse and keyboard, but if that's not your style use a controler, doesn't matter which one

I've been digging the controller for Shadow of Mordor and Witcher so I'll go with it :D
