I've seen a few refits without one. Definitely some of Aer Lingus's ATRs, and I think, some recent easyJet or Ryanair narrowbodies.

That'll never go away. Planes have crashed because passengers smoked in the lavatory and threw the cigarette into the trash, causing a fire.
