What if there really was a significant risk that people would subsequently die because of dependencies in their infrastructure. In such a case it couldn't be tested.

You'd have to balance it against the risk of a real prolonged outage. Sometimes even the worst test outcome is better than not testing.

Local tests can be done with GPS jammers. It would make sense to start with those and build up from there.
