I may be expressing myself badly here. The point is *not having to go to a hospital* to get routine or urgent care, but just being able to go to a doctor. That is becoming almost impossible in rural America, and is a different (though connected) problem to overall lack of care

And I began this by saying expansion would help, not solve everything. But to the problem you raise above, I think direct state aid is necessary: subsidize education and salary for providers willing to work in rural areas. Which is I think consonant with Matt’s original idea.
