We still don't have a satisfactory answer for why poor megacities like Dhaka, Lagos, Jakarta, Manila, or New Delhi haven't had a runaway coronavirus outbreak like the ones that hit New York or Lombardy, while other parts of the world are on their second or third iteration.

Maybe. Project forward 6 months: the articles saying "Here's What We Wish We Knew Back When COVID Hit" are probably going to point to several enablers and associated mitigation measures. Those 3 are currently my best guess. We'll see.

If you look at the history of infectious disease with complex patterns, it's usually an extremely simple single cause. Yellow fever is a good example that baffled everyone; scurvy is another.
