If you drive through rural America, you'll find that health care is concentrated in the cities, sometimes to such an extent that it appears to be the only industry there. The failure to tie this topic in to public health and the coming pandemic tonight is remarkable.

A huge factor causing that is the ACA. The structure of the ACA encourages consolidation, which leads to less profitable locations in rural areas being closed and having services narrowed. Lots more at play but switching to M4A reduces pressure to consolidate.
