Okay, so about this stupid consent must be enthusiastic suggestion. Here's a partial list of things that might make this problematic (but which do not rule out people consenting to a sexual encounter):

Enthusiasm as a standard rules out encounters where consent comes from pressure, eg your boss demands sex (and maybe they'll fire you if you refuse). What do you think of this?

Another factor (perhaps trivial compared to the ones you list but fairly widespread) is simply introversion. For some of us, quiet contentment with life’s pleasures is more than sufficient…

Of course, people low in extraversion can easily be misread as unfriendly or uninterested, and it does help to make one’s intentions and attitudes clear, but that’s a more general issue that goes well beyond consent for sexual activity.
