Could you please explain the requirements for the “Super Ben” achievement? Is this story related or are there specific requirements? After the patch, it’s still locked for me. I’ve already completed the game pre patch and didn’t know if I need to complete another play through

There are NPC characters that randomly appear around the world which trigger a combat event. You need to defeat the enemies and save 10 of these NPC characters to unlock the achievement.

Thanks for this patch, It's fixed all achievements for me! I just wanna ask you, is it possible to get all upgrades in one playthrough? I'm missing 3 health upgrades for my last achv, must i do another playthrough?

It is possible to get them all through one playthrough. Make sure you have all the main, side, passive, Void Camps and Combat Challenges completed. There are also secret ones hidden around the world that are not on the map.
