Since 2015 bank connections to global money network SWIFT hacked in Ecuador, Vietnam, Bangladesh, & Mexico; at least $93m stolen. Banks & SWIFT often try to keep these hacks secret; this just counts thefts for which the news has leaked.

You should look into banks asking other banks to return fraudlent wires. The receiving bank is not obligated to return funds but occassionaly will return funds as a measure of good will (read: help another bank and not the recieving entity).
