Just read on modder forum and tried the skins myself. Underwear is not worth it, most people run with cloths on, and RPers use other kind of mods anyway :P navel piercing based on race styles are a thing!

WoW has a crazy mapping of skins and items. They wont fit for all races, because the character model coverage of character skins is very different on each race and the item skins are just pasted on them, which is the reason why items look very different based on the race.
