Ideally, the tests would be conducted in conjunction with a news organization (e.g. NYT, WaPo, etc.) which would select a nonpartisan team of doctors to administer the tests and help interpret the results for the public.

He's not saying they're constitutionally required to do it. He's suggesting they should volunteer for it. It's like saying I can't play baseball because it's not mentioned in the Bill of Rights.

Why just the older candidates?

Presumably because cognitive function degrades, eventually, as you age. That doesn't necessarily mean this is a good idea (or that just out and testing all the candidates is a bad one, if you want to go that route).
