Why do we still get asked for this at Dr surgeries etc? Presumably it's because it's easier than searching for a name as a unique identifier. What is the best way to deal with it? "Can you look me up by my patient number?"

Ask to be processed privately in a side-room. If TSA can do it (and they will, never had a problem asking) then the doctors’ surgery can, too. If they kick off, just say very politely, “Sorry, but I value my privacy, and you should too.”
