Generally in older cats it's Thyroid issues. Have you noticed if his coat is greasier?
Ok I just spoke with the cat doc, he weighs 8.2 pounds but has lost muscle mass. It could be thyroid, gut, diabetes, or cancer. He also has a heart murmur and arthritis in his right knee. 
Yes he was a fat boy a little over a month ago.
You are doing everything right and some more tests will help narrow it down (early on there are always many possibilities) wait to hear what the Vet has to say about diagnostics. I am a Vet student/Nurse so can help with any questions. Sending good vibes over to you and kitty
