Mrph. Part of the logic, I think, is that the posited authorities decide for everyone, and don't take individual variation into account (because they can't do that the way the individuals themselves can). They'll impose a treatment that will kill you sooner than let you choose.

Suppose a treatment saves %large and kills %tiny. The authorities make it mandatory. This accomplishes their goal of "universal imposition that costs fewest lives", but is obviously worse than letting people who are in %tiny decide…
