My grandfather was a chemical process engineer, and I wound up with a bunch of his old books when he passed. One of them was a compendium of industrial accident reports and lessons learned. What always got me was that every report noted the victims were “overcome” (by fumes) 1/

But the way they worded it made it sound like a trashy romance novel. “Operator, realizing a wrench was left in the pressure vessel, hurriedly defeated the LOTO device with an angle grinder, entered the vessel, and was overcome (With emotion upon being reunited with his wrench)”
